Your First Visit
Please arrive 15 minutes before your scheduled appointment this will allow us time to finish the paperwork and discuss any financial questions you may have concerning your initial appointment. Please bring the following items with you to your first appointment:
- New Patients Information Sheet
- Dental Insurance Card
- Identification such as Driver’s license, military ID, or state ID
Our staff will provide you with certain documents needed for patient files and to ensure you are receiving the best dental care possible in North Ogden.
