Ben Lomond Dental is contracted with a number of major dental insurance providers. Our team knows the ins and outs of dental insurance benefits, and they are happy to file and track your claims to help you make the most of your benefits.
Please be sure to bring a copy of your current insurance card to each appointment, so that we keep your records as up-to-date as possible.

Choice Dental Plan
Our “Choice Dental Plan” is specifically designed for individuals and families who are currently uninsured and seeking a dental plan that would save them money and meet their individual needs without dental insurance.
How does the Choice Dental Plan work?
The dental plan requires the participant to pay an annual premium that allows for two preventative appointments a year. The two preventative appointments include a dental exam and cleaning, one set of bitewing x-rays, and fluoride. Those patients requiring periodontal cleanings (beyond a regular preventative cleaning) can apply half of the annual premium towards two periodontal services. A savings of 15% will also be applied on all other dental procedures offered in our office including cosmetic services such as teeth whitening, veneers, etc. These types of services are typically not covered by traditional dental plans making the Choice Dental Plan a better option for your dental needs.
With its unique approach, the Choice Dental Plan offers tremendous opportunity and savings for our patients. The plan has no annual maximum or deductible, no waiting periods, and prior authorizations are no longer needed. The dental office is also no longer burdened with claim forms and other time-consuming and costly paperwork. This enables our patients to receive dental treatment when it is needed.

COST TO SUBSCRIBER
Adult enrollment fee (per person, 18 years of age and older) $340.00
Child and/or additional family enrollment fee (per child) $300.00
The annual premium is non-refundable if a patient chooses to enroll in a traditional dental or any other discount plan. In this event, the Choice Dental Plan would be considered null and void.
Contract Length: 1 year from date of enrollment
Periodontal Services
**Periodontal services often require more than two annual visits per year. Half of the annual premium can be applied towards two periodontal cleanings, exam x-rays, and fluoride. Any additional visits will receive the 15% savings.
Exclusions to the Plan
**Sedation, products (electric toothbrushes, Peridex, Prevident, Theramints, whitening gel, etc.) and services requiring specialty care including lab costs outside of our office.
